INSURANCE

We are participating in some, yet not all HMO networks. You must contact your insurance prior to your visit to verify your mental health coverage.

We’ll need your insurance details when you first reach out to us. While we try our best to check your insurance coverage before your appointment, it’s important for you to confirm that every provider you visit is included in your plan.

 

Depending on your health insurance or employee benefit plan, your services might be fully or partially covered. Get in touch with your provider to understand how your plan handles payments for behavioral health services, and check if mental health claims are sent to an out-of-network insurance company. Here are some questions to guide you:

  1. Does my plan cover behavioral health services?
  2. How does my plan compensate me for these services?
  3. Are mental health claims sent to a third-party out-of-network insurance company?

Keep in mind that many insurance plans may have a copayment or co-insurance that you need to pay. Please be prepared to pay estimated deductible amounts, co-insurance, and copayments at each visit.

QUESTIONS FOR YOUR INSURANCE PROVIDER

  • Does my health insurance plan include mental health benefits?

  • Is this provider covered under my plan?

  • Do I have a copay? If so, what is the amount?

  • Do I have a deductible and if so, what is it and have I met it yet?

  • Do I have co-insurance? If so, what this the percentage that I will be responsible to pay?

  • Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?

  • Do I need written approval from my primary care physician in order for services to be covered?

  • How does my plan pay for an Out-of-Network Provider?

  • How do Superbills work and how do I submit them? What is the amount reimbursed for office visits?

     

 

OPTION TO SELF PAY

If you prefer, we can give you a Superbill after your appointment. You can then submit this to your insurance company for reimbursement. The payment you make initially will be applied to your deductible. Once your deductible is fulfilled, you’ll receive reimbursement for most of your appointment cost. Feel free to reach out if you have any questions.

We accept various forms of payment, including Visa, Mastercard, American Express, Cash, and Check. Payments are required at the time of each visit.

 

SUPERBILLS

A Superbill is a receipt for the services you’ve paid for, containing essential information needed by your insurance company for reimbursement. It’s strongly recommended to contact your insurance company and inquire about the specific procedure for submitting a Superbill, as the process may vary between insurance providers.

 

CANCELLATION POLICY 

Kindly provide us with a minimum of 24 business hours’ notice if you need to cancel or reschedule your appointment. We allocate this time specifically for you. If you cancel or miss your appointment without adequate notice, a cancellation fee will apply. To avoid the fee, please call us at 770-746-7071 at least 24 business hours before your scheduled appointment. We appreciate your cooperation in this regard.
